InSANITARIAN – Inilah sejarah baru transformasi digital bidang kesehatan. Hari ini, 26 Juli 2022, Kementerian Kesehatan RI mencatatkan perubahan cukup positif terkait transformasi dan integrasi data layanan kesehatan nasional lewat platform digital bernama “SatuSehat”. Inilah awal dimulainya transformasi bidang kesehatan di era digitalisasi.
Hal itu dilakukan Kementerian Kesehatan sebagai upaya menghadirkan layanan kesehatan yang lebih baik melalui transformasi digital. Kementerian Kesehatan Republik Indonesia (Kemenkes RI) meluncurkan Indonesia Health Services (IHS) sebagai platform transformasi dan integrasi data layanan kesehatan nasional dengan nama “SatuSehat” di Hotel Raffles Jakarta, Selasa (26/7).
“Hari ini secara resmi kita memperkenalkan “SatuSehat” sebagai nama Indonesia Health Services yang merupakan platform integrasi dan standardisasi layanan data kesehatan di Indonesia,” kata Menteri Kesehatan (Menkes), Budi Gunadi Sadikin dalam pidatonya.
Menjadi salah satu upaya Kemenkes RI dalam mentransformasikan layanan kesehatan melalui digitalisasi, “SatuSehat” merupakan sebuah platform konektivitas data, analisis, dan layanan untuk mendukung integrasi antar aplikasi dan fasilitas pelayanan kesehatan (fasyankes). Nantinya, pasien tidak perlu berulang-ulang mengisi formulir baru saat berpindah fasyankes.

Melalui “SatuSehat”, pasien juga bisa mendapatkan informasi mengenai kondisi kesehatannya secara lebih transparan. Karena resume rekam medis di rumah sakit dicatat dan direkam secara digital dengan aman melalui persetujuan (consent) pemilik data.
“Hal ini sejalan dengan rencana Kemenkes RI dalam mentransformasikan PeduliLindungi menjadi aplikasi kesehatan masyarakat. Jadi manfaatnya akan semakin berkembang dari yang semula untuk penanganan pandemi bertransformasi menjadi lebih luas lagi,” jelas Menkes.
Chief of Digital Transformation Office (DTO) Kemenkes RI, Setiaji menerangkan bahwa platform integrasi ini dibuat melalui proses yang panjang. Mulai dari perencanaan, jajak pendapat dari para ahli hingga uji coba fase alpha dan beta dengan peserta dari beragam latar belakang institusi. Seperti rumah sakit, laboratorium, health-tech, farmasi, klinik mandiri, praktisi hingga akademisi.
“Platform ini telah diuji coba kepada 41 rumah sakit vertikal milik pemerintah pada tahap alpha testing dan sedang berlangsung uji coba fase beta yang melibatkan 31 institusi dari latar belakang yang berbeda–beda,” kata Setiaji.
Lanjut Setiaji, berbagai aspek pun tak luput jadi perhatian Kemenkes RI pada setiap proses pembangunan platform ini. Mulai dari aspek teknologi, regulasi, keamanan sistem dan privasi hingga hal-hal pendukung lainnya. Hal ini dilakukan untuk menjamin keberlangsungan sistem hingga perlindungan data pribadi masyarakat sebagai pengguna.
Pada acara ini juga ditunjukkan secara langsung (live demo) gambaran bagaimana proses pertukaran data pasien melalui integrasi “SatuSehat” dengan Sistem Informasi Manajemen (SIM) yang dimiliki rumah sakit maupun laboratorium dengan aplikasi PeduliLindungi.
“Kita tunjukkan dengan platform ini, journey pasien menjadi jauh lebih cepat ketika berobat di rumah sakit maupun saat melakukan pengecekan kesehatan di laboratorium. Dengan teknologi, semua jadi lebih efisien dan transparan,” ujar Setiaji.
Setiaji menjelaskan saat ini sudah ada 16 fasyankes yang telah terintegrasi dengan “SatuSehat”. Empat belas diantaranya merupakan peserta dari fase alpha testing, yaitu 10 rumah sakit umum daerah (RSUD) di DKI Jakarta dan 4 rumah sakit vertikal milik Kemenkes RI.
“Harapannya hingga akhir tahun 2022 ini, terdapat 8.000 fasyankes yang terintegrasi dengan “SatuSehat” dan target seluruh fasyankes terintegrasi di tahun 2023 mendatang,” tutup Setiaji.
Transformasi Kesehatan
Peluncuran platform digital bernama “SatuSehat” ini, tidak lain sebagai rangkaian rencana Kementerian Kesehatan melakukan transformasi kesehatan. Sebelumnya, Menteri Kesehatan RI Budi Gunadi Sadikin menginisiasi adanya transformasi di bidang kesehatan. Ia telah menetapkan ada 6 jenis transformasi yang akan dilakukan, yakni transformasi Layanan Primer, Layanan Rujukan, Sistem Ketahanan Kesehatan, Sistem Pembiayaan Kesehatan, SDM Kesehatan, dan Teknologi Kesehatan.
“Kita melakukan enam transformasi pertama adalah layanan primer ini yang paling penting di promotif preventif, yang kedua adalah transformasi layanan rujukan rumah sakit, ketiga transformasi sistem ketahanan kesehatan ini kalau ada pandemi lagi supaya kita lebih siap dari sisi obat-obatan, alat-alat kesehatan, tenaga kesehatan cadangan itu masuk ke sana, termasuk surveilan terhadap penyakit menular kita ingin pastikan baik lokal, nasional, maupun regional itu harus siap,” katanya, Selasa (31/5).
Menkes Budi melanjutkan transformasi keempat adalah transformasi sistem Pembiayaan Kesehatan. Hal ini sebagian besar ada di BPJS, namun ada juga asuransi swasta dan harus dipastikan bahwa ini sustainable. Transformasi kelima adalah SDM Kesehatan dan keenam adalah transformasi Teknologi Kesehatan, ini terkait teknologi informasi dan bioteknologi.
“Itu adalah 6 kerangka besar yang akan kita kejar sampai 2024,” ucapnya.
Transformasi Layanan Primer
Menkes Budi mengatakan saat ini ada sekitar 12 ribuan Puskesmas yang tersebar di semua wilayah Indonesia. Ia menilai jumlah tersebut tidak akan mencapai pemerataan pelayanan kesehatan. Ada sejumlah program yang akan dilakukan di antaranya menata ulang jaringan fasilitas layanan kesehatan.
Dirinya akan merevitalisasi Posyandu agar menjadi lebih formal dengan anggaran yang sesuai. Nantinya Posyandu ini bisa diatur oleh Kementerian Dalam Negeri atau Kementerian Desa, Pembangunan Daerah Tertinggal, dan Transmigrasi. Posyandu ini akan bertindak secara lebih aktif bukan hanya melayani bayi dan ibu tapi akan melayani seluruh siklus hidup termasuk remaja, dewasa, dan Lansia.
Menkes Budi juga menilai perlu mereformasi laboratorium kesehatan masyarakat.
“Jadi setiap Puskesmas bisa melakukan layanan laboratorium misalkan 100 kali tes, kemudian di atasnya laboratorium kesehatan kabupaten/kota, di atasnya lagi laboratorium provinsi, kemudian regional, dan nasional,” ucap Budi.
Transformasi Layanan Rujukan
Transformasi ini akan dimulai dengan tiga penyakit penyebab kematian paling tinggi di Indonesia yaitu penyakit jantung, stroke, dan kanker.
Sebagai contoh untuk penyakit jantung, masalahnya tidak semua provinsi memiliki rumah sakit dengan fasilitas untuk pasang ring di jantung.
“Data yang saya miliki dari 34 provinsi yang bisa melakukan operasi pasang ring itu hanya 28 provinsi. Terus kalau pasien sudah dipasang ring juga tidak bisa, maka tindakan berikutnya adalah bedah jantung terbuka, ini jumlahnya turun lagi dari 28 provinsi kalau tidak salah ke 22 provinsi,” tutur Menkes Budi.
Dikatakan Menkes Budi, pihaknya punya target bahwa rumah sakit di seluruh provinsi pada 2024, harus bisa melayani penyakit jantung, stroke, dan kanker.
“Akses layanan dan standar layanan tertentu untuk jantung, stroke, dan kanker saya mau rata tersedia di seluruh provinsi,” ucap Menkes Budi.
Setiap rumah sakit dengan dokter yang berprestasi, lanjutnya, akan ia pertemukan dengan dokter dari negara lain untuk menjalin kerja sama. Sedangkan dokter-dokter yang terbaik dari luar negeri akan didatangkan ke Indonesia untuk meningkatkan kapasitas dokter Indonesia.
Ketahanan Kesehatan
Menkes Budi mengatakan dirinya ingin memastikan bahwa vaksin diagnostik dan terapeutik itu semuanya ada di Indonesia. Minimal 50% itu diproduksi di dalam negeri dari hulu ke hilir.
“Kita ingin memastikan sudah bikin rencananya nanti ini lebih berlaku untuk teman-teman di Farmasi dan bidang industri. Jadi kalau mereka melakukan produksi dalam negeri semua government procurement, akan berikan prioritas ke mereka,” tutur Budi.
Pembiayaan Kesehatan
Yang akan dilakukan terkait transformasi pembiayaan kesehatan adalah melakukan transparansi dan perhitungan yang bagus. Hal itu untuk menghindari terjadinya masalah antara penyedia jasa dan yang membayar jasa.
